Allow adding a document to the search results from Staged and Saved documents.

• Display a different icon & id for semantic search document in the upload popup
• Integrate the functionality added with #6 (closed) to Staged Documents and My Documents.
• Show an error/warning message when the selected document is missing from the search results, signifying that the chosen document is no longer available in the index.
