update subnet list so it always creates 3 public and 3 private subnets as described in the readme; formatting caught by pre-commit

* Fix missing IAM role path variable

The missing IAM role path is causing a plan with this module to fail.
This value is accepted by the IRSA module, so reinstate it and pass to
the module via the IRSA configuration.

* Add other missing values

The namespace is also referring to an incorrect value. The eks cluster
id also appears to be required.

* Remove iam role path as requested
